Program/Event Assessment Form

This form is to be completed within 10 days of the funded event. Along with completion of this form groups must bring all of their receipts, invoices and other necessary documentation to the Student Government Office and give them to the UFB administrative advisor, Charlene Walker. The office is located on the 6th floor of the Steger Student Life Center.
